Community-Based Family Support Programs From CONCERN
CONCERN Professional Services for Children, Youth, and Families runs a group of community-based programs meant to help keep or reunite children with their families and community, and they are provided in a variety of community settings rather than at one office. The work pairs family strengths with proven methods that support and promote quality parenting. CONCERN provides community-based services throughout Pennsylvania.
The programs are:
- Visit Coaching, which identifies and attends to the needs of the children, plans for the individual needs of each child, strengthens a parent's coping skills, and builds skills in behavior management, child development and parenting
- Intensive Family Reunification Services, short-term, intensive, family and home-based work to reunite families with children who are in out-of-home placement as quickly and safely as possible, and to serve families with children at home by reducing the need for placement
- Parent Education Programs using two evidence-based models, Triple P (the Positive Parenting Program), which gives simple and practical strategies to build strong, healthy relationships, manage children's behavior and prevent problems, and Nurturing Parenting, which teaches parents and caregivers how to promote and nourish themselves, their children and their environment
- Casework Services to empower parents and help them maintain the family unit, which can include housekeeping, education, budgeting training, transportation to appointments and locating appropriate housing; these can run for a short period, for an emergency, or for an extended period, with the frequency and duration of sessions set by need
- Family Finding, in which a caseworker locates and engages as many family connections as possible, such as relatives and friends, to create a support network for a child and family
- Family Group Decision Making, an evidence-based model that helps parents become the leaders in decisions about their children's safety, permanency and well-being. This one is offered to families whose children are at risk of out-of-home placement or families with open case management with CYS who need to identify additional supports.
- Supervised Visitation Services, where parents and children visit in a supported, nurturing and supervised setting, with emphasis on positive relationship building and parent-child bonding
- School Engagement, for children and families impacted by truancy, aimed at increasing consistent school attendance. These services can last between two to six months, include school and home visits, and involve working with the school, county caseworkers, juvenile probation and district magistrate offices.
- Transportation Service for clients and their families who are actively involved in Children and Youth Services, used for reasons such as court attendance, visitation, drug and alcohol or mental health appointments, emergency placements, and getting children to and from school
- Intensive Case Management for transitional youth in subsidized apartments, emphasizing full-time employment, access to educational and vocational programs, and eventual transitioning to permanent housing
- School Based Resource Coordinator, which connects children, youth and families to resources that increase academic participation and performance, with individualized goals built with the student, family, school staff and community resources. Services run an estimated 3-6 months, with a possible continuation after a reassessment.
- Crisis Rapid Response, which coordinates a family meeting within 24 hours of a Crisis referral and within 72 hours of a Rapid referral, for families whose children are at risk of out-of-home placement, bringing the family and their existing supports together to help resolve the risk factors. Family Finding strategies are used to identify and invite connections to the meeting.

Two descriptions do point to county involvement: Transportation Service is for people actively involved in Children and Youth Services, and Crisis Rapid Response starts from a Crisis or Rapid referral.
