Youth Specialized Community Navigator

Centro Hispano Daniel Torres in Reading, PA is hiring a Youth Specialized Community Navigator within the Community Impact / Youth Violence Prevention Team, reporting to the Director of Community Impact.

The position is non-exempt and grant-funded, with continuation dependent on grant renewal or continued funding. Compensation is $43,000–$53,000 annually, and the schedule is full-time, in-person, 35 hours per week including 1-2 evenings per week.

The Navigator acts as a trusted advocate, mentor and connector for youth and young adults ages 13–24 who are impacted by violence or at elevated risk, delivering trauma-informed, culturally responsive, strengths-based support to interrupt cycles of violence and promote positive outcomes.

Approximately 60% of the time is spent on direct support and case navigation for the FUERZA program:

• maintaining an active caseload with regular contact and documentation,
• conducting assessments,
• building individualized support plans,
• goal-setting and progress monitoring,
• advocacy and life-skills coaching,
• targeted outreach to marginalized at-risk youth,
• connecting youth to education, housing, counseling, workforce and legal aid services with accompaniment when appropriate,
• responding to crises and safety concerns per organizational policy and mandated reporting rules,
• documenting case notes, referrals and outcomes per grant requirements,
• and participating in case reviews with the Lead Youth Specialized Community Navigator.
