Youth Specialized Community Navigator (Youth Specialist Community Navigator)

Centro Hispano in Reading, PA seeks a Youth Specialized Community Navigator for its Community Impact / Youth Violence Prevention Team, reporting to the Director of Community Impact.

The role is non-exempt and grant-funded, with continuation dependent on grant renewal or continued funding. Compensation is $43,000–$53,000 annually.

Schedule: full-time, in person, 35 hours per week, including 1–2 evenings per week.

The Navigator acts as advocate, mentor, and connector for youth and young adults ages 13–24 impacted by violence or at elevated risk, providing trauma-informed, culturally responsive, strengths-based support to interrupt cycles of violence and promote positive outcomes.

Duties

Direct support and case navigation in the FUERZA program (about 60%)

• consistent trauma-informed mentorship through scheduled contacts, goal-setting and progress monitoring
• maintaining an active youth caseload with regular contact, documentation and follow-up
• conducting assessments, developing individualized support plans, advocacy and life-skills coaching toward personal, educational, employment and wellness goals
• targeted outreach to marginalized, at-risk youth for rapport-building and recruitment
• connecting youth to education, housing, counseling, workforce and legal aid services with case navigation and accompaniment
• responding to crises and safety concerns per organizational policy, mandated reporting and referral protocols
