U.S. Passport Applications at the Reading Public Library
The Main Library of the Reading Public Library is a passport acceptance facility, which means you can fill out a U.S. passport application and hand it in there in person. Passport service is by appointment only.
Passport hours are Monday and Tuesday from 10AM to 6PM, Wednesday, Thursday and Friday from 10AM to 5PM, and Saturday from 9:30AM to 3:30PM. Book through the "Make An Appointment" link on the library's passport page at readingpubliclibrary.org/us-passport-service, which opens a booking calendar at calendly.com/rplpassport/us-passport-service. Appointments must be made in the applicant's name. Say when you book if you need a passport photo taken. The library also offers stand-alone photo services without an appointment.
All applicants, including children, must be present. For a child under 16, both parents must be present, or the absent parent must provide a notarized Statement of Consent form (DS-3053). Children ages 16 and 17 apply in person with one parent or guardian present, and both the child and the adult with them need appropriate ID. A child's passport must be renewed in person using form DS-11. Passports for children under 16 are valid for 5 years, and passports for applicants 16 and older are valid for 10 years.
What to bring: - The passport application form, filled out before your appointment in medium to heavy black ink with no white-out, and not signed until an agent asks for your signature. Forms are available on-site or online at pptform.state.gov. - Proof of U.S. citizenship: a previously issued, undamaged U.S. passport (it may be expired), a certified birth certificate issued by the city, county or state with the parents' full names, a Consular Report of Birth Abroad or Certification of Birth, a Naturalization Certificate, or a Certificate of Citizenship. If both parents are not listed on the birth certificate, you will need to apply for a new birth certificate. - Proof of identity, plus a copy of it on 8 1/2 by 11 inch paper: a previously issued, undamaged U.S. passport, a Naturalization Certificate, a valid driver's license, a current government ID from a city, state or federal agency, or a current military ID for military members and dependents. A 16 or 17 year old without a valid driver's license can bring a learner's permit, a school ID, a state ID, a current government ID or a current military ID. - One color passport photo, 2 by 2 inches, taken within the past 6 months and showing your current appearance, full face front view on a plain white or off-white background, with 1 inch to 1 3/8 inches from the bottom of the chin to the top of the head, in normal street attire. Camouflage and uniforms are not permitted. - Two forms of payment, because the government fee and the library fee must be paid separately.
The application fee goes to the Department of State by check or money order and depends on the type of service requested. For adults age 16 and older it is $130 for a passport book or $30 for a passport card. For minors age 15 and under it is $100 for a child passport book or $15 for a child passport card. Passport cards are not for travel by air; they are valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean only.
The library charges a $35 execution fee per person for applicants using form DS-11, and $15 per person for photos taken at the appointment. Library fees can be paid by cash, check, money order, debit or credit, and the library fees for everyone in your group can be combined into one payment.
The Main Library is at 100 S. Fifth St., Reading, PA 19602. The library's general number is 610-655-6350 for questions. The elevator at the Main Library is out of service, which affects accessibility for library services. If you need accommodations because of the elevator outage, call Vicky Fuller at 610-655-6365 to arrange your appointment.
For current processing times, check the Department of State website at travel.state.gov. The status of your application should be available online about 2 weeks after you apply at travel.state.gov, or you can call the National Passport Information Center at 1-877-487-2778, or 1-888-874-7793 for TDD/TTY, with questions about a completed application or a travel emergency. If the State Department mails you a letter asking for more information, review the letter and contact the phone number listed in it. Because of privacy rules, the library cannot contact the State Department on your behalf.
