Supports Coordination for Intellectual Disability and Autism From The Arc Alliance
The Arc Alliance provides Supports Coordination services to people of all ages with an intellectual disability and/or autism diagnosis, and to children diagnosed with complex medical conditions. A Supports Coordinator helps you find services and supports, navigate the service system, watch over your health and safety, and monitor the services you receive once you are enrolled in the intellectual disability and autism service system. The work is person centered, aimed at the person having an everyday life in their community.
Eligibility is based on having an intellectual disability and/or autism diagnosis before age 22, or being a child under age 21 diagnosed with complex medical conditions.
To register, first call your county's intellectual disabilities office and ask for an intake. In Berks County that number is (610) 236-0530. If you qualify, the county gives you a list of willing and qualified Supports Coordination Organizations that serve your county. You pick one and tell the county your choice. Once you are registered with that organization, a Supports Coordinator will contact you to set up a meeting to talk about your needs and goals. You can change your Supports Coordinator or organization at any time: to request a different coordinator, contact the SC Supervisor or the SCO Director, and to change organizations, contact your county's intellectual disabilities office and ask for an SCO transfer.
If you need paid services, your coordinator can help you seek Intellectual Disability Waiver funding. The waiver is a Medicaid program that pays for needed services. It is needs based rather than automatic, and it is the payer of last resort, so other funding has to be used up first. There is a waiting list for waiver funding. Your coordinator meets with you and your team to decide what services are needed, writes a PUNS (Prioritization of Urgency of Need for Services), sends the county the paperwork it requires, which usually covers medical, functional level, financial and other areas of life, and helps you find providers and schedule tours. The county reviews everything and decides who receives a waiver; the page says the timeline varies greatly depending on need, system budget and available waiver spots. If you are approved, the services chosen are added to the ISP and sent to the county, and your waiver begins when the ISP is approved and the start date arrives. Keep in touch with your coordinator so they know about changes and can advocate for waiver funding.
While you wait for a waiver, or if you do not have one, the page points to natural supports such as family, friends, neighbors and agencies like OVR for employment services or United Way; to alternative waivers through other offices, such as the Community HealthChoices waiver through the Office of Long Term Living, which The Arc Alliance does not handle directly but can give general information and contacts for; and to contacting your local legislator to advocate for waiting list funding, with information at thearcpa.org. You can still receive Supports Coordination services without a Medicaid waiver.
The Arc Alliance also keeps a resource list on its website covering benefit programs, financial assistance and mental health supports, and it is open to anyone whether or not they are registered with The Arc Alliance. If you need a resource that is not listed, contact your coordinator or The Arc Alliance.
