Early Intervention for Babies and Toddlers From Service Access and Management
Service Access & Management, Inc. runs Early Intervention Service Coordination, which helps support families and meet the needs of infants and toddlers, birth to age 3, who have a developmental delay or disability. Early Intervention services can be provided in the home or local community.

To start, complete the Early Intervention Referral Form at sam-inc.org/ei_referral. The form asks for the referral date, the county where the child lives, who is making the referral and their relationship to the child, a phone number, the child's name, date of birth and address, a parent or guardian name and phone, insurance information and the reason or concern behind the referral. A parent or family member can make the referral, and so can a physician, a hospital or NICU, childcare, a shelter or another agency. Files such as a PDF or a photo can be attached, up to 10 MB each.
