Berks Encore Care Plus Life Care Managers for Older Adults and Families

Berks Encore Care+ connects older adults and their family members with a Life Care Manager who helps them sort out care decisions when aging feels hard to navigate. Berks Encore, a nonprofit leader of senior services in Berks County, describes Care+ as a holistic, client-centered approach to caring for older adults, and says the guidance and expertise of its Life Care Managers help families take action and make decisions that ensure quality care and an optimal life for those they love, which reduces worry, stress and time off work for family caregivers.

A Life Care Manager is a health and human services specialist who acts as a guide and advocate for families who are caring for older relatives or disabled adults. They are educated and experienced in fields related to aging, which can include counseling, gerontology, mental health, nursing, occupational therapy, physical therapy, psychology or social work, with a specialized focus on issues related to aging and elder care.

Berks Encore suggests it may be time to contact a Care Manager when caregiving for an aging family member becomes overwhelming. Reasons listed include:

  • The person you are caring for has limited or no family support.
  • You live at a distance from your loved one.
  • The person you are caring for is unable to live safely in his or her current environment.
  • Your family is burned out or confused about care solutions, has limited time or expertise with chronic care needs, or is at odds about care decisions.
  • The person you are caring for is not pleased with current care providers and requires advocacy, or is confused about his or her own financial or legal situation.
  • Your family needs education or direction about dementia behaviors or about available services.

The first phone consultation with a Life Care Manager is free. From there, a Comprehensive Assessment and Personalized Care Plan can be developed for a fee, and follow-up engagement for the Care Plan can be arranged at hourly fees. Fees are set on a sliding scale based on the senior's income.

To get started, call a Life Care Manager at 484-577-4243, or complete the Care+ referral form on the Berks Encore services pages at berksencore.org. The form asks for the client's name, address and phone, who is making the inquiry and their relationship to the client, the reason for the call and immediate concerns, what assistance the family already provides, how the person is managing at home, the medical insurance provider, income, and what supportive services are needed, such as meals, transportation to appointments, groceries or legal help.
