ProBerco Companion and Habilitation Support for Adults With Disabilities
ProBerco's Family Support Services send a Direct Support Professional to work one to one with an adult with a disability in their own home and in the community. The supports are for people living independently in their home or residing with their parents or caregivers. Direct Support Professionals provide emotional support as well as socialization and community engagement assistance. What each person receives is determined by their Individual Support Plan.
Companion Services provide supervision for adults with disabilities to keep them safe, healthy and well in everyday life. A Direct Support Professional may help with activities of daily living and encourage the person to take part in community activities based on their own choice and self-determination. The hours of service are determined by the needs of the individual and may include a few hours each day or a few hours each week.
Habilitation Services are the supports a person needs to live an everyday life at home, whether living alone or with family. A Direct Support Professional helps a person gain the skills to live as independently as possible and may teach how to use community resources, including public transportation, meal planning, exercise and financial literacy. The work follows the goals already set in the person's Individual Support Plan.
Family Support Services also includes respite services, which are mainly a break for a caregiver. These supports are sometimes known as in-home and community services, because the help is provided in a person's home or in the community.
