HIV Navigation and Prevention Help From Co-County Wellness Services
Co-County Wellness Services runs HIV Navigation Services, called HNS, for people living with HIV or at high risk of getting it. Counselors link people at high risk to health care systems, assist with health insurance and transportation, identify and reduce barriers to care, and tailor health education to the person to influence health-related attitudes and behaviors.
HNS also guides people to and keeps them in prevention services, including HIV risk behavior change interventions, PrEP and nPEP, and services that deal with other major risk-complicating factors such as substance use, violence, stigma, mental health, homelessness and other health disparities. The stated goals are achieving life goals, increasing healthy feelings, thoughts and actions, developing daily routines to stay healthy, and protecting your health and the health of others by reducing behaviors that can lead to HIV and STI infection or re-infection. The work also builds emotional awareness, problem solving, goal setting, assertive communication and self sufficiency.
Care and prevention services are for anyone at risk of becoming infected with HIV or living with HIV. All prevention services are free for residents of Berks and Schuylkill County and are available in English and Spanish.
There is also a more structured version called Individual HIV Navigation Services, designed for people 16 and older who are HIV positive or at a high risk of contracting it due to their behaviors. The core is 5 one-hour sessions with a trained counselor to motivate long term behavior change by developing your ability to manage stress and deal with it in a healthier way. More sessions can be added after the first 5. Sessions are one on one, non-judgmental and conversational, with weekly goal setting, clear thinking techniques, relaxation, SMART problem solving, communication, positive self-talk, arguing against negative thoughts and managing emotional discomfort. Each session is client-centered and meant to fit the individual.

The Reading office at 429 Walnut St. is open Monday, Wednesday, Thursday and Friday from 9am to 5pm and Tuesday from 11am to 7pm. In the event of inclement weather, the office follows the Reading School District closing schedule. If you are living with HIV, case managers can also help you choose and get to a health care provider who specializes in HIV, and nurses who are AIDS Certified Registered Nurses can help you understand HIV medicines and build a plan for taking every dose.
