HIV Case Management and Medication Help From Co-County Wellness Services
Co-County Wellness Services provides HIV case management, help choosing an HIV doctor, and nurse support with HIV medicines for people living with HIV in Berks and Schuylkill counties.
Its care and prevention services are for anyone at risk of becoming infected with HIV or living with HIV. All care services are free to anyone who is living with HIV and is a resident of Berks or Schuylkill County.
A case manager gives you information about HIV disease, connects you to medical care so you can live well with HIV, and makes sure you know about the supportive services you may be eligible for. If you are living with HIV, a case manager can work with you to make an action plan for living well with the virus, based on goals that are important to you and using your personal strengths. Case managers know about HIV disease, the challenges of living with it, the special services for people living with HIV, and the other agencies in the community that can help you.
Case managers can also help you think about what matters most to you in choosing a health care provider, such as how knowledgeable they are about HIV and whether they accept your health insurance, and can refer you to local providers who specialize in HIV. You can get case management no matter who you choose as your health care provider. Co-County Wellness Services has an agreement with the Center for Public Health, a Ryan White funded clinic at the Reading Health System, that allows its case managers to work as part of your care team if you are a patient there.
For medicines, the nurses are AIDS Certified Registered Nurses, which means they have had special training about HIV and HIV medicines. They can help you understand how HIV medicines work and what choices you have, and help you make your own plan to take every dose every day. This matters because missing doses can give the virus a chance to change so that the medicine will not work as well.

Office hours there are Monday, Wednesday, Thursday and Friday 9am to 5pm, and Tuesday 11am to 7pm. The Schuylkill County office, Schuylkill Wellness Services, is at 512 N. Centre Street, Suite 1B, Pottsville, PA 17901, general phone (570) 622-3980, with office hours Monday and Tuesday 9am to 5pm, Wednesday 9am to 6pm, and Thursday and Friday by appointment.
In the event of inclement weather, the Reading office follows the Reading School District closing schedule and the Pottsville office follows the Pottsville School District closing schedule.
