Care Coordination for Opioid Addiction From Reading Hospital
Reading Hospital runs an Opioid Use Disorder Center of Excellence, where care coordinators help people with an opioid addiction get connected to treatment and to the other things they need to stay in recovery. The Pennsylvania Department of Human Services designated Reading Hospital as a Center of Excellence in 2016. Centers of Excellence offer team-based services focused on the "whole person," with the explicit goal of integrating behavioral health with primary care.
It is for people who have an opioid-related substance use disorder, who may also have behavioral and physical health conditions at the same time, who need help navigating the care system, and who need guidance to stay engaged in treatment to avoid relapsing. Opioids include pain medications that doctors prescribe, which can be addictive and can also be obtained illegally on the street, and illegal drugs like heroin.
Care coordinators connect people to medical and treatment services, including substance abuse treatment, mental health care, primary care, medication-assisted treatment for addiction, pain management, dental care, and eye care. They work with you to build a plan for unmet social needs such as housing, childcare, education and job skills, employment, transportation, clothing and food, health insurance, and recovery-friendly recreation and activities. The care management team can also help you build a health recovery support network, give education, information, and support about addiction and recovery to you, your family, agencies, and community groups, help you move to your next level of care, help you rebuild family and other important relationships, and meet with you out in the community.
