Mental Health Case Management for Adults From Awaken Mental Health

Awaken Mental Health, a service of Treatment Access and Services Center (TASC), Inc. of Berks County, provides mental health case management for adults, and this part of its site sets out the referral criteria it uses to decide who can be accepted.

To be referred, you must be 18 years of age or older and have a diagnosis within DSM IV R (or succeeding revisions thereafter). People whose principal diagnosis is mental retardation, psychoactive substance abuse, organic brain syndrome or a V-Code are excluded.

You also need to meet at least one of these treatment history criteria:

  • Currently being discharged from a PA State Hospital
  • 6 or more days in an inpatient psychiatric unit during the past 12 months
  • Met criteria for an Involuntary Civil Commitment (302) at least once in the past 12 months
  • Currently receiving or in need of mental health services and receiving or in need of services from 2 or more human service agencies or public systems such as Drug and Alcohol, Vocational Rehabilitation, Criminal Justice, and others
  • Refusal of or inability to adhere to medication regimen and/or missing 3 or more appointments with a mental health provider in the past 6 months
  • 2 or more Crisis contacts in the past 12 months
