Mental Health Case Management for Adults From Awaken in Reading
Awaken Mental Health Case Management, a service of Treatment Access and Services Center (TASC), Inc. of Berks County, supports individuals living with mental illness to attain access to medical, social, educational, and other services to promote a more healthy and stable lifestyle. A case manager helps you connect with the care and supports you need so you can develop and maintain healthy behaviors that encourage independent living.
Awaken provides mental health case management as approved and overseen by OMHSAS/Community Care Behavioral Health (CCBH) standards, and it is Medicaid/Community Care Behavioral Health approved. Co-occurring disorders, mental health and substance abuse together, are also accepted.
To be accepted you must be 18 years of age or older and have a diagnosis within DSM IV R (or succeeding revisions thereafter). People whose principal diagnosis is mental retardation, psychoactive substance abuse, organic brain syndrome or a V-Code are excluded.
You also need to meet at least one of these treatment history criteria:
- Currently being discharged from a PA State Hospital
- 6 or more days in an inpatient psychiatric unit during the past 12 months
- Met criteria for an Involuntary Civil Commitment (302) at least once in the past 12 months
- Currently receiving or in need of mental health services and receiving or in need of services from 2 or more human service agencies or public systems such as Drug and Alcohol, Vocational Rehabilitation, Criminal Justice, and others
- Refusal of or inability to adhere to medication regimen and/or missing 3 or more appointments with a mental health provider in the past 6 months
- 2 or more Crisis contacts in the past 12 months
People come into the program by referral, and awakenmentalhealth.org has a Referral Form page along with the referral criteria. Awaken is currently accepting new referrals, and there is a waitlist, so it can take time before staff connect with each individual.
